Toy Request Form

Ayer PD The Ayer Police Department has a limited number of toys available for our residents in need. We are presently accepting toy request applications through Friday December 6, 2019.
The program is open to Ayer parents/guardians of children living in Ayer aged infant to twelve. Preference will be given but not limited to families currently receiving government assistance (e.g. Mass Health, WIC, SNAP, EBT, SSI, and etc.).

Photocopies of the following must be submitted to process your application:

  1. A valid government ID and current utility bill to establish Ayer residency
  2. Children’s birth certificates,
  3. Proof of government assistance benefits (not required, but will be given preference).

Toys will be given away by appointment on Saturday December 7, 2019. The number of toys given per child is dependent on donations received by the Department.
